If you are planning to travel to Europe now or in the very near future, the European Union (EU) is requiring all visitors to fill out that they are calling the EU Digital Passenger Locator Form or EU PLF before they enter whatever country they are intending to visit. For most travelers, this means it must be filled out before you board your flight to Europe. Each country has their own form, see below. This list is meant as a guide only. Always check for the most up to date information.
What Is the Form?
The form is a way for European countries to contact you or perform contact tracing for people that may have come into contact with Covid-19.
How Much Does It Cost?
The form is free!!! There are third party companies that will fill out the form for you for a fee.

How Do I fill Out the Form?
Depending on the country you are traveling to, you will have to either fill out the form online or download it. Some countries have a short form and others are longer. Most countries require you to register to complete the form.
You may be sent a form with a QR code and or a PDF version of your form, save it to your smart phone. You may have to upload it to your airline reservation or show it during boarding.
Do It At Least 48 Hours In Advance!
Most countries require you to complete the form at least 48 hours in advance. If you don’t complete these forms, your airline may deny you boarding and your vacation will be ruined. Plus, if you have travel insurance, it won’t reimburse you for failing to complete the forms. We recommend you complete the forms two weeks in advance to avoid problems.
Do I Need a Negative Pre-Departure Negative Covid Test?
This depends on the country you are traveling to. Most countries do require a test within 48-72 hours of departure. Check with the airline or country you are visiting before departure.
Other Notes
Again, the situation is always changing, so make sure you check the airline and country website before you leave your home country. Nearly all countries are requiring a proof of vaccination certificate. If you have a digital copy, this is best. But a paper copy should also be just fine. You will just have to ensure the vaccination you received is on the list of approved providers. Bring plenty of masks too.
What About A Country I Am Transiting Through?
Even if you never leave the airport, you are subject to the transiting country’s rules and regulations. Fill out the form just in case.
